New Patients
At this time we are accepting new patients on a limited basis. Please complete the online medical history below, once we receive this form, we will contact you to schedule an appointment.
Your initial visit will include updating radiographs , a consultation with a dentist and a thorough cleaning. If additional dental services are required, a treatment plan will be discussed with the dentist and additional appointments may be scheduled.
