To get started, please complete the online medical history form below. Once we receive your completed form, a member of our scheduling team will contact you directly to schedule your first appointment.

Your First Visit

Your initial visit will typically include necessary radiographs, a consultation and examination with one of our dentists, and a thorough dental cleaning. If additional dental treatment is recommended, your dentist will discuss your treatment plan with you and additional appointments may be scheduled.

We are happy to submit claims to other dental insurance plans as an out-of-network provider. For questions regarding your specific coverage, benefits, or out-of-pocket costs, please contact your insurance provider directly.
